Built by Consolidated at San Diego. Assigned to the US Navy (USN). Assigned to Patrol Wing 10 (Pat Wing 10) to squadron 22 (VP-22). Assigned squadron number 13. No known nickname or nose art.

Mission History
On February 5, 1942 while taking off from Saumlaki Seaplane Base in Saumlaki Bay, this Catalina grounded on a coral reef and beached. Afterwards, it was abandoned as not salvageable.
