Built by Curtiss in Buffalo, New York. Delivered to the U. S. Army serial number unknown. No known nose art, nickname or squadron number. When lost, engine and weapon serial numbers unknown.

Wartime History
Assigned to the 13th Air Force, 347th Fighter Group, 68th Fighter Squadron. No known nickname or nose art.

Mission History
On February 27, 1943 took off from Henderson Field on Guadalcanal at 4:15pm on a mission to the northwest over Vella Lavalla. Engaged in a dogfight near Vella Lavalla, last seen crashing into the sea. When this aircraft failed to return it was listed as Missing In Action (MIA).

Memorials
Lewis was officially declared dead the day of the mission. He is memorialized on the tablets of the missing at Manila American Cemetery.
