Built by Kawasaki. Delivered to the Japanese Army Air Force (JAAF) as a Type 3 Hein / Ki-61 Tony manufacture number unknown. Assigned to the 78th Sentai. No known markings.

Mission History
On July 24, 1943 took off on a mission over Bena Bena (Benabena). Over the target area, shot down by ground fire and crashed nearby.

After the crash, the wreckage of this aircraft was located by Australian forces. Possibly, Ki-61 Tony crashed near Goroka is this aircraft.
