Aircraft History
This Avenger was damaged by anti-aircraft fire on July 14, 1943 while piloted by Lt. Alfred F. Jacobsen, 124104 while based on Guadalcanal. It was repaired. Later, assigned to CASU-10 based in Espiritu Santo.

Mission History
On a local area flight from Espiritu Santo, this Avenger failed to return and crashed.

Wreckage
Wreck discovered in 2000 on Espiritu Santo, and is an MIA site.

Memorials
Crew was declared dead the day of the mission. Memorialized on the tablets of the missing at Honolulu Memorial Cemetery (Punchbowl).
